Hello Everybody!
My name is Abdellah and I'm a quality responsible in a packhouse for
fresh vegetables and fruits and we are aiming for BRC food standard by
the end of this year, i would like if there's someone who can help me
with example of sheet concerning return to work after illness or
holiday!!

Welcome to the forum.
I am writing to you from the angle of HACCP which is predominantly part of BRC/IOP. Anyway, someone will write in if I am on the wrong path.
1. On your question of someone coming back to work from holidays, are you suspecting or know for a fact that he has travelled to some countries or regions where there was a known outbreak of transmittable disease or recently been to an orchard farm that has been known to have been attacked by some bugs that are dangerous to your line of trade for fear of cross-contaminations. If in doubt, practise what the Custom Department does - "Quarantine" the potentially affected personnel from work until verified by a doctor that he is free from any known transmittable disease. Otherwise, the normal personnel hygiene check list should be enough.
2. Your second question is perhaps of more concern. If a personnel has been known to be suffering from illness and has been cleared by the doctor or hospital, to return to work then a medical report is presumably available to verify the person's state of health.
Generally, a medical leave is given to a person for general illness (where I come from) and an indicated "return date" is given for the worker to return to work. Thats for general illness.
In case of severe illness, you may want to make sure that the personnel has indeed fully recovered from it. A post medical report confirming his clearance is therefore the only verification document.
