I do not understand what you are asking for - are you asking for a QA Pre-Op Inspection form?  And if so, what kind of operation - seafood is pretty broad.

If you mean daily sanitary checks per CFR 21 part 123 then a good place to start is with the Sanitation Control Procedures for seafood.
