FDA Requirements for Master Case Label?

Hello, does anyone know if there are a specific set of requirements set by the FDA for what information needs to be on master case labels? Master case meaning there will be multiple retail units in one master case to be shipped from a co-packer. I presume it would be at least the following:

 

Product Name

Item Number

Lot Code

BBD

Net Weight

Unit Count

Company Name

You're on the right track, but essentially it boils down to Identity, declaration (allergens, additives, etc.), traceability which include units/weights. There are additional state regulations as well, such as plant number for example.

I am not seeing anything specific in the Food Labelling Guide...do you have any information on the state regulations in california?

Note that the FDA Labeling Guide is from 2013 and does not include the latest revisions to nutrition facts panel requirements.  I hope FDA provides an updated guide soon.
