Traceability of rework (Polystyrene and Pulp)

Hello,
I am looking for information or insight as to how to trace rework in a Polystyrene & Pulp manufacturing facility; we produce to-go containers, plates bowls etc.
Any info would be great.

Context on how you process would help.
Initially, these are some questions that may guide/help you.
1. How do you handle your PS that is for rework? Do after production, is any left over that you consider for rework is labelled with atleast qty, batchcode and when generated?
2. Do you regrind PS? If yes, do you have records to reflect it?
3. For your PS addition, is the bin separate for virgin or for rework? Do you record every tipping?
Depending on the process, above may suffice or may require further info.
