Maybe a few pages missing ? eg cleanliness / cleaning procedures, rodents ? insects ? environment, foreign body control ?
Of course it may depend a bit on the OP's specific ultimate objective also (if any) . BRC ? (if yes, 1-2 checklists available)

Remember, the template needs to reflect YOUR food safety management system.
Don't add questions to the template if your manual does not specify the requirement.

The attachment-checklist in this post based on US / cGMP regulations is quite good IMO (maybe will need shortening for yr specific situation). Also see following posts in same thread.
